Controlled Drug Register Nigeria: What Pharmacy Software Should Record

A controlled drug register is not a stock spreadsheet. Here is what Nigerian hospital and retail pharmacies should demand in HMS, and what to test in a demo.

DawaHQ Clinical Team• Hospital Operations & Product9 min read

The Monday inspector does not ask whether your HMS has a pharmacy module. They ask who issued pethidine last night, to which patient, remaining balance, and which pharmacist signed. A controlled drug register is that book. Software that only decrements paracetamol stock is not a register.

This guide is for superintendent pharmacists, medical directors, and administrators buying pharmacy inside HMS or EMR in Nigeria. Pair it with PCN premises records, pharmacy software, and ward MAR. Score the register on a named controlled item, not a slide titled “NAFDAC compliant.”

What is a controlled drug register in a Nigerian pharmacy?

In operational language, the register is the attributable history of restricted medicines:

  • Which product. Name, and identifiers you actually store (batch where you track it, NAFDAC number where you record it).
  • Class label. The schedule or local class your premises SOP uses for that SKU. That is a label on the item, not a government certificate printed by the vendor.
  • In and out. Receipts, issues, returns, and write-offs, with a running balance.
  • To whom. Patient when the issue is a dispense; ward, theatre, or destruction when it is not.
  • By whom. A named pharmacist login, not “Pharmacy” as a shared tablet.
  • When. Server time, not a backfilled Sunday evening.

Buyers mash four different artefacts into “the CD register”:

  1. Dedicated register screen. A list of controlled dispenses: date, drug, class, patient, quantity, balance, dispenser, notes.
  2. Stock-movement ledger. Every restock, dispense, return, expiry write-off, and adjustment for that SKU, controlled or not.
  3. Hardcover statutory book. The paper CD1/CD2-style register some premises still keep because their superintendent or inspector expects ink.
  4. Ward MAR. What the nurse marked given at the bed. That is not pharmacy issue.

If a vendor says “we have a controlled-drug register,” ask which of those four they just opened. Then make them flag one item, dispense it, and show both the register screen and the stock ledger.

DawaHQ treats this on the pharmacy solution as an add-on you enable per clinic: controlled flag on the drug, a Controlled Drug Register screen under Pharmacy, and a stock-movement ledger for every quantity change. There is no separate “PCN module” SKU. Do not buy a compliance add-on that cannot show the same prescription, invoice, and named login as the rest of the HMS.

How is a digital register different from the hardcover CD book?

Superintendent pharmacists already know the paper failure modes. Vendors still demo an Excel export.

Paper fails as parallel books (main pharmacy, theatre, ICU), missing signatures, and a volume that went home with the locum.

Digital fails differently: shared pharmacy logins, stock adjusted with no reason, controlled items treated like ordinary analgesics, and a pretty register page that never receives a row after dispense.

A useful digital register keeps chronology, a named actor, and a running balance, then adds search, role locks, and a link to the prescription or walk-in issue. It does not print a statutory CD1 or CD2 form unless the vendor shows that form. DawaHQ does not ship a government CD1/CD2 template. If your SOP still requires a hardcover book, keep it. Do not run two conflicting balances.

Walk-in stock dispense and prescription-item dispense are both real. On DawaHQ, a walk-in can take an optional typed patient name; a prescription dispense carries the patient from the order. An empty patient column on a restricted issue is a SOP gap, not a software badge.

What should each register entry record?

Ask for fields staff already write in the book.

| Field | Why it matters | |-------|----------------| | Date and time | Inspection windows are periods, not “recent.” | | Drug name | The SKU issued, not a category blob. | | Class / schedule label | Clinic-configured class on the item. Do not treat a vendor badge as Nigeria’s official schedule list. | | Patient | Named person for outpatient and inpatient issues. Optional free text on a walk-in is weaker than a chart link. | | Quantity issued | Units leaving the cupboard. | | Balance after | Running balance. A list of issues with no balance is a diary, not a register. | | Dispensed by | Named login. Shared “Pharmacist” destroys the row. | | Notes | Wastage, partial pack, overnight explanation. | | Prescription link | When the issue is against an order, not a verbal request. |

DawaHQ’s register screen shows date, drug, class label, patient, quantity, balance after, dispenser, and notes, with search on drug or patient and clinic-scoped rows. Optional NAFDAC number lives on the drug master (tracking, not “NAFDAC compliance”). The page loads a recent window (the UI asks for the latest hundred). It does not list the prescription id on that table. Ask for a dated inspection export in the demo rather than assuming a PDF.

Silent edits look like concealment even when honest. Prefer a reason on adjustments and expiry write-offs. DawaHQ records stock movements for restock, dispense, return, expiry write-off, and adjustment. That ledger is the quantity story. It is not dual-pharmacist witness. DawaHQ does not ship a second-pharmacist sign-off on controlled issues.

Who should be allowed to open the register?

Curiosity browsing of controlled issues is a professional and privacy problem. Named accounts beat a laminated password on the hatch.

In DawaHQ, Pharmacy (including the Controlled Register nav) is meant for owner, clinic admin, and pharmacist when the pharmacy add-on is on. Pharmacists can dispense and manage inventory. Doctors prescribe; they should not be the night dispenser by default. Nurses mark the ward MAR; they do not own the pharmacy register. Cashiers take money; they should not open the pethidine book. See staff roles.

The register API is clinic-scoped and gated on inventory capability. Sidebar still hides it from roles that are not owner, clinic admin, or pharmacist. Store manager is a different seat: hospital store is consumables, not the CD book. Deactivate departed locums: locum access.

Does software make you PCN or NAFDAC compliant?

No. PCN premises licensing is organisational and professional. Software can make records easier to show. It cannot grant the licence. Soften any vendor who says “PCN compliant pharmacy software” to “records a superintendent can defend.” Same for NAFDAC: storing a registration number on the SKU is tracking. It is not a NAFDAC certificate for the clinic or the product.

DawaHQ stores an optional NAFDAC number on the drug, flags the item as controlled, and can show a class label when you (or a template) set one. Stock dispense blocks expired pack dates you recorded on the item. It does not block dispense because a NAFDAC registration string looks stale. Do not buy “expired-NAFDAC auto-block.”

NDPA. Dispensing history is health data. Prefer NDPA-oriented consent logging and a patient-access audit trail for who opened charts. DawaHQ is built to align with the Nigeria Data Protection Act 2023 on those controls. We do not sell “NDPA certified,” “fully NDPA compliant,” or “HIPAA pharmacy.” Filing, a DPO where required, and a rule against curiosity browsing remain yours. See patient consent and NDPC registration.

NDHI / FHIR / NHIA portal. A hospital register is not national exchange. HMO claim workflows after a billed dispense are not NHIA e-submit. See HMO reconciliation.

Optional pack barcode at dispense is a clinic setting (off by default). It is a pack-match at the counter, not bedside closed-loop, not FEFO lot pick. Multi-lot FEFO remains on the roadmap; clinics manage expiry with current batch fields today.

How do pharmacy, ward MAR, and theatre stock stay distinct?

Matrons and pharmacists already know this split. Vendors blur it.

Pharmacy register answers: did this restricted pack leave the pharmacy, against which order or walk-in, with which balance.

Ward MAR answers: did the nurse give that dose at this bed. Stock leaving the hatch does not prove the 2 a.m. dose reached the patient.

Theatre / ICU consumption burns controlled items quickly. Issues to a case or admission need a documented path. DawaHQ’s theatre and critical-care modules are separate enablements. Do not assume floor-stock acknowledgement is the same row as the outpatient window.

Mental health and dental premises still hold restricted items. The register does not disappear because the clinic is procedure-light. See mental health EMR and dental software.

What should we test in a controlled-drug demo?

Bring the superintendent pharmacist, not only the cashier.

  1. Enable pharmacy if it is off. Confirm the Controlled Register item appears for pharmacist / owner / clinic admin, and not for cashier.
  2. Open a drug. Flag it as controlled. Record a NAFDAC number if you use one. Confirm a class label if a template or your catalogue already set one (the stock-dispense banner shows it when present). The add-drug form flags controlled; it is not a government schedule picker.
  3. Dispense a quantity to a named patient (prescription path if that is your night workflow). Confirm the controlled banner at stock dispense.
  4. Open the Controlled Drug Register. Confirm date, drug, class, patient, quantity, balance after, and dispenser. If the row is missing, open stock movements for that SKU and write down the gap. A labelled screen that does not receive the issue is not a register.
  5. Restock the same SKU. Confirm the movement is not a silent quantity bump.
  6. Try an expired pack date on a stock dispense. DawaHQ should refuse that path when expiry is recorded.
  7. Ask for dual-pharmacist witness and a CD1/CD2 print. If they are not there, decide whether paper remains your statutory book.
  8. Log in as cashier. Confirm they do not get the register nav.
  9. Show the billed line on the same patient if the item has a selling price. “Issued free at the window” should not be an invisible write-off.
  10. Ask who exported the last hundred rows, and how you would pull a dated inspection window without WhatsApping the vendor.

Allergy and Five Rights checks on the prescription-dispense path are safety at the counter. They are not the register. Do not call the allergy matcher “AI.”

What to do next

List this week’s restricted SKUs: opioids, benzodiazepines, codeine combinations, theatre induction agents, any item your SOP treats as controlled. If two people share the pharmacy login, or the hardcover book and the software disagree on balance, you do not have a register yet.

Then book a demo or request access. Walk one flagged item, one dispense, one register row, and one stock-movement line on the same morning. Explore pharmacy, HMS for Nigeria, and EMR for Nigeria. Pricing is sales-led from ₦25,000/month, then a scoped quote. If the product cannot name the pharmacist on the issue and show a running balance, it will not help when the question is asked in writing.
