Medical Certificate Software Nigeria: Sick Leave Notes Tied to the Chart

A sick-leave note is not a Word letterhead. Here is what Nigerian hospital software should record, who may issue it, and what to test in a demo.

Friday at reception is a stack of requests that are not consultations: write a sick leave, fit for duty, excuse the child from school. If the answer is a Word template on a shared desktop, the clinic has a documentation gap, not a stationery problem. Medical certificate software in a Nigerian EMR or HMS exists so the note, the examination, and the named clinician share one record.

For medical directors, administrators, and OPD leads. Pair it with staff roles, discharge summaries, and who opened the chart. Score the certificate on a named consultation that actually prints. This is operational practice, not legal advice, and not an MDCN form catalogue.

What is a medical certificate or sick leave note in a Nigerian hospital?

A medical certificate is a short clinical statement that someone was examined and is either unfit or fit for a named period. Employers, schools, and sometimes insurers ask for it. It is not a discharge summary, not a medico-legal report, and not a death certificate.

Buyers mash four documents into “sick leave software”:

  1. Sick-leave / work-excuse note. Unfit for duty from date A to date B, issued after an examination.
  2. Fitness letter. Fit for duty as of a date. Different question, same family of paper.
  3. School excuse. A paediatric or school-health variant of (1), not an immunisation card.
  4. Full medical report. History, findings, tests, opinion. NSITF-style comprehensive reports and court papers live here. They are not the OPD sick note.

If a vendor says “we do certificates,” ask which of those they opened. Then make them issue one from a saved consultation and print it.

On DawaHQ this sits on the core consultations chart, not a separate SKU. From the consultation, Medical Certificate opens the form (if none exists) or the printable page and clinic-generated code (if one does). Issuing requires the consult capability.

Who may issue a sick leave certificate under MDCN rules?

Software roles are not a practising licence.

MDCN guidance is blunt: documents the law expects a medical practitioner or dental surgeon to sign (including sick leave) are not valid if someone who is not fully registered issues them. The Medical Director still owns who may treat and who may sign. An HMS cannot print an NMA seal, look up the public register, or invent an official form number. We do not publish MDCN form codes because the product does not ship them.

What the product does gate is the consult capability. On DawaHQ, the default matrix gives consult to owner, clinic admin, doctor, dentist, optometrist, physiotherapist, and mental-health practitioner. Nurses, receptionists, cashiers, pharmacists, and laboratory staff do not get it. The certificate API (POST /api/certificates) returns 403 without that capability. Staff-aware RLS keeps rows inside the clinic.

That software gate is wider than MDCN’s signing rule. Clinic admin can consult in the product. A physiotherapist can consult on the physiotherapy module. Neither fact turns them into the registered practitioner who should sign a sick-leave note required by law. Put a clinic SOP on top: the examining doctor or dentist issues the note, then stamps the empty “Official Use Only” box. Do not buy “the EMR is MDCN-compliant certificates.”

Named logins still matter. A shared “Doctor” password on the certificate is the same failure as a shared SOAP. See locum access when the weekend cover is a different human.

What should hospital software record on the certificate?

Ask for fields staff already fight over on letterhead.

| Field | Why it matters | |-------|----------------| | Patient identity | Name plus chart identifiers (patient code, date of birth, gender when recorded) | | Clinic identity | Name and address on the print, not a blank page | | Certificate type | Sick leave, work excuse, school excuse, fitness, or other. Demo sick leave and fitness first | | Period | Unfit from / unfit to, with an inclusive day count. End date cannot precede start | | Diagnosis policy | Optional diagnosis, and a switch for whether it prints. Default should be off (“on medical grounds”) unless the clinician chooses otherwise | | Recommendations | Rest, limited duty, follow-up. Optional, clinician-authored | | Issuer | Logged-in clinician name, locked, not a free-typed “Dr. whoever” | | Clinic code | A reference the front desk can read back. On DawaHQ this is MC-{year}-{four digits}, uniqueness retried, not an MDCN serial | | Print path | Browser print from the saved record. Confirm it, do not assume a filed PDF in the document store |

Useful extras on DawaHQ: consultation diagnosis can pre-fill the form; a consultation-plan checkbox can create a sick-leave period with diagnosis hidden when dates are entered; WhatsApp opens a short text summary (name, code, dates, issuer) via wa.me. That summary is staff-initiated. It is not official delivery, not the patient portal, and not a substitute for print plus stamp.

Do not expect, today: QR verification against the MDCN register, a generated NMA stamp, the clinician’s registration number on the print (staff records can store a number; the certificate page does not print it), a revoke workflow, a digital signature, or a stored PDF in patient_documents.

How is a sick-leave certificate different from fitness, school excuse, or discharge?

Sick leave / work excuse. Unfit for duty from date to date. The print banner is UNFIT FOR DUTY, with the inclusive day count.

Fitness. Fit for duty as of the start date on the same date fields. Different clinical question. Do not reuse a sick-leave letter and hope HR notices.

School excuse. Same family of note, often for a child. It is not an immunisation printout. Vaccine cards belong with paediatric EMR.

Discharge summary. Course, TTO, follow-up, bed leave. A discharge is not a two-day malaria excuse. See discharge summary EMR.

Referral letter. A focused ask to another facility. Related paper. Not a certificate.

On DawaHQ the consultation-plan trigger always writes type sick_leave with diagnosis hidden. Use the dedicated certificate form when you need fitness, school excuse, work excuse, or a printed diagnosis. The consultation page shows Issued plus the code once a row exists for that consultation.

Does software make the certificate legally valid for an employer?

No.

MDCN / NMA. Validity stays with the registered practitioner, the examination, the signature, and usually a stamp. Software stores the statement and prints it. The empty stamp box is honest: you still stamp.

Employer / Labour Act. Many workplaces ask for a doctor’s note after an absence. That is HR policy. DawaHQ does not ship an employer portal, a statutory Labour Act form, or payroll integration. Do not buy “HR-compliant sick leave.”

NHIA / HMO. A certificate is not a claim batch and not an NHIA national portal. See HMO claim workflows.

Medico-legal and NSITF reports. Comprehensive injury reports and court papers are different work. The product type other is a label, not those templates.

NDHI / FHIR / “national EMR.” A private HMS print is not a national exchange. Do not accept those phrases as a substitute for a named clinician and a dated period.

What about NDPA, diagnosis privacy, and WhatsApp?

Health records are sensitive personal data. Printing a diagnosis on the employer’s copy when the clinician did not intend that is a controller mistake, not a feature.

Prefer NDPA-oriented consent logging and a patient-access audit trail. DawaHQ is built to align with those controls. We do not sell “NDPA certified” or “fully NDPA compliant.” The form defaults show diagnosis to off, so the print says “on medical grounds” unless the clinician turns the switch on. That is a privacy-supporting default, not a completed NDPA programme. See patient consent.

The staff DSAR JSON does not currently include medical_certificates. If a data-subject pack must list issued notes, say so in the demo and keep an off-system copy rule until that table is in the export. Patients cannot download a certificate from the portal today.

WhatsApp share is a convenience text with a patient name. Treat it like any other staff message. Clinic reminder credentials are a different product (WhatsApp clinic reminders).

What should you test in a medical-certificate demo?

Run this morning’s OPD request, not a brochure.

  1. Open a named consultation as a doctor. Tap Medical Certificate. Confirm the form, not a Word download.
  2. Leave diagnosis off. Set unfit from/to. Generate. Confirm the print says UNFIT FOR DUTY, “on medical grounds,” clinic name, locked doctor name, and MC- code.
  3. Print (browser print). Confirm the empty official-use box. Stamp it in the demo if that is your SOP.
  4. Repeat on a fitness type. Confirm FIT FOR DUTY.
  5. As a receptionist or cashier, confirm they cannot issue (403 or no consult path), matching staff roles.
  6. Ask whether clinic admin or allied-health consult seats can hit the same API. On DawaHQ they can. Write your issuing SOP beside that fact.
  7. Confirm registration number and NMA stamp are not auto-printed. If the vendor claims a live MDCN lookup, make them show it.
  8. Confirm the path: book a demo, request access, custom quote. No self-serve trial.

Bring the Medical Director and the OPD lead, not only IT. If they will not examine the patient before the note prints, the field list does not matter.

How does DawaHQ support certificates today?

On clinics that run the hospital EMR/HMS consultations module (core):

  • Types on the form: sick leave, work excuse, school excuse, fitness, other. Demo sick leave and fitness first; confirm school and work excuse persist in your environment.
  • Consult-capability API (GET/POST /api/certificates), clinic-scoped, 403 without consult.
  • Dates, day count, optional diagnosis, print switch, recommendations, notes. Doctor name locked from the signed-in user.
  • Printable page with clinic name/address from the signed-in user metadata, patient identifiers, unfit/fit banner, optional diagnosis, recommendations.
  • Consultation-plan shortcut that creates a sick-leave row with diagnosis hidden when start and end dates are present.
  • WhatsApp text share of a short summary. Browser print. No portal download. Not in the DSAR JSON today.
  • Qualified wording: a clinic-generated certificate tied to a consultation. Not an MDCN form, not NMA-stamped, not employer-certified, not NDPA/NHIA/NDHI.

Same HMS as billing and HMO claim workflows. Pricing is sales-led from ₦25,000/month, then a scoped quote.

What to do next

Count last week’s letterhead notes: unsigned Word files, diagnosis spilled to HR by habit, and receptionists typing “Dr.” for a doctor who already left.

Then book a demo or request access. Walk one consultation → certificate → print, one fitness letter, and one receptionist 403. Explore HMS for Nigeria, EMR for Nigeria, and the hospital solution. If the product cannot show the examination and the note on the same patient, it will not help when the employer calls back.
